Hope & Healing Counseling is required by the Health Insurance Portability and Accountability Act (HIPAA) to maintain the privacy of your protected health information ("PHI"), provide you with this notice of our legal duties and privacy practices, and follow the terms currently in effect.

We collect information you provide directly — such as your name, contact details, insurance information, clinical history, and session notes — as well as information generated through the course of your care, including diagnoses, treatment plans, and billing records.

We may use and disclose your PHI without your written authorization for:

• Treatment — coordinating your care among providers involved in your treatment.
• Payment — billing you, your insurer, or a third party for services rendered.
• Health care operations — quality assessment, staff training, and practice administration.

Other uses and disclosures — such as sharing information with family members, sending marketing communications, or responding to legal proceedings — generally require your written authorization, except where required by law (for example, to prevent serious harm, or in response to a valid court order).

Your rights

• Right to access — request a copy of your medical and billing records.
• Right to amend — ask us to correct information you believe is inaccurate or incomplete.
• Right to an accounting of disclosures — request a list of certain disclosures we have made of your PHI.
• Right to request restrictions — ask us to limit how we use or disclose your information.
• Right to confidential communications — ask us to contact you in a specific way or at a specific location.
• Right to file a complaint — with our office or with the U.S. Department of Health and Human Services, without fear of retaliation.
